INSURANCE:

We will be happy to help you file your insurance claim forms
without additional charge as a courtesy to you. Please
remember, your insurance benefits are determined by your employer or
the plan that you have selected. Our office will do
all we can to help you receive your maximum benefits, but your
insurance policy is an agreement between you and your insurance
company, not our office.
Insurance usually does not pay 100% of treatment costs.
It will be your responsibility to pay any amount not covered by your
benefits at the time of service. We will make every
attempt to estimate your personal financial obligation, but benefits
can vary greatly from one policy to another and changes can occur in
your policy without our knowledge. It is your
responsibility to notify our office of any changes in your policy.
The insurance company will not notify us. This
includes changes in your employment status, your carrier, insurance
dollar maximums and calendar year dates. Please let
us know if you have used any insurance benefits at another dental
office this year.
